Your First Visit
When you join our practice, we like to begin with a complete dental examination. This initial appointment will be spent discussing your dental history, creating a treatment plan and getting to know you before we schedule you for any other services such as hygiene or restorative care. Your first visit with our hygiene staff will be a separate appointment.
Payment for services
We request payment in full at the time of service. In appreciation of your prompt account settlement we offer a 5% discount for payment by cash, check or credit card. We are also happy to bill your dental insurance company for you once you become an established patient. Please bring insurance information with you to your first appointment.
Failed appointments
If it becomes necessary to cancel an appointment, we ask that you notify us at least 24 hours in advance. Confirmation calls are made one to four days before your appointment and we are happy to contact you at your most convenient number and/or email address. Failed appointments are costly to a practice as this time has been saved specifically for you; therefore, our policy is to charge for missed appointments.

Should you decide to choose our office for your dental care needs, please call us at 360-385-4700 and ask for Juanita, or email records.uptowndentalpt@olympus.net to set up your initial exam.
It is very helpful for us to have your treatment records and x-rays from your previous dentist in advance of your initial exam, so that we have your records in-office when you arrive. Please send the completed New Patient Health History and HIPAA forms prior to scheduling your initial exam appointment.
To print the new patient forms, simply click the links below to open them and choose "Print" from the "File" menu.
